NOTE:

1. The annual membership fee will be invoiced on acceptance of membership and will be calculated on a pro rata basis from date of application.

2. The membership fee is reviewed on an annual basis.

3. The above figures are inclusive of Value-added Tax (VAT).

4. All cheques are payable to SAMED.

5. Subscription fees must be fulfilled should a member resign before the end of the SAMED financial year.
